I hereby enroll my child/myself to The Ballet School of Colombo (TBSC) and do hereby agree to abide by the Rules of the School set out below:

1. I understand that participation in dance classes can and may result in possible personal injury to my child/myself despite all precautions being taken by TBSC and all its teachers, dancers and staff members. By ticking the box hereto I assume all risk related to any and all activity within and outside TBSC before, during or after class.
2. I agree to release from responsibility TBSC including all teachers, dancers and staff members from any liability for any personal injury to my child/myself whether caused as a result of dancing or otherwise, including but not limited to; scrapes, bruises, cuts, sprains, fractures, broken bones, concussions, demise or damage or loss to any personal property, that may occur on the premises before, during or after class.
3. I voluntarily agree to take full responsibility for the conduct and safety of my child/myself before and after class and when outside the premises of TBSC and I agree to release all teachers, dancers and staff members from any responsibility for the conduct and safety of my child/myself before and after class or outside the premises of TBSC.
4. I understand that payment in full is required to access classes and agree to make payment at the beginning of each term. I understand that failure to make payment on or before the requested time frame for a duration of 2 terms will result in the denial of admission to classes and cancellation of enrollment. (Should a student wish to rejoin, a full year's fee payment is required upon re-joining.)
5. Any previous TBSC student who wishes to rejoin after a break of more than 3 terms needs to re-enroll by paying both the Term Fee and Admission Fee.
6. Once fees are paid for the allocated program, they are non-refundable. Payments are for the intended term/month and may only be transferred upon special request and approval by the admin team. The full term fee is required regardless of vacation days or other absences.
7. Students must attend the requested days per program and wear the proper TBSC uniform at all times during class sessions.
8. I hereby certify that the enrolling student is in good health and is capable of participating in physical activities.
9. I acknowledge and agree that, while enrolled at The Ballet School of Colombo, the student will not attend or be enrolled in ballet classes or programmes at any other ballet school, dance studio, or establishment. This condition applies only to ballet training and does not restrict participation in other programmes. Failure to comply may result in the student's enrolment being terminated.
